  New Patients

Register as a new patient in our Patient Portal. Within one business day, our office manager will contact you to schedule your first appointment, discuss what to expect, review insurance details, and process a $25 booking fee. This fee will be applied toward your visit cost and is refundable if you cancel at least two business days prior to your scheduled visit.

Please register using your legal name and gender as listed on your ID or insurance. We understand this may be uncomfortable, and we sincerely apologize for the limitations of current medical systems. ♥
