  1. I understand that the payment plan is designed to distribute the total cost of services into manageable monthly payments.
  2. I will enroll in the payment plan during my first appointment.
  3. I acknowledge the payment is a recurring charge of $99 per month, including months when I do not have an appointment.
  4. I understand that I cannot cancel or pause my monthly payments between appointments, and my care services will be put on hold if I fail to make my payments due for services rendered.

  New Patients

Register as a new patient in our Patient Portal. Within one business day, our office manager will contact you to schedule your first appointment, discuss what to expect, review insurance details, and process a $25 booking fee. This fee will be applied toward your visit cost and is refundable if you cancel at least two business days prior to your scheduled visit.

Please register using your legal name and gender as listed on your ID or insurance. We understand this may be uncomfortable, and we sincerely apologize for the limitations of current medical systems. ♥
