  1. I understand that the subscription plan is designed to distribute the total cost into manageable monthly payments.
  2. I will enroll in the subscription plan before my first appointment.
  3. I acknowledge the subscription is a recurring charge of $79 per month, including months when I do not have an appointment.
  4. I understand that I cannot cancel or pause my subscription between appointments, and my care services will be put on hold if I fail to make my subscription payments.
